Wellness Incentive

What is the Wellness Incentive?

CREATION Health Employees empowers you to take charge of your health by participating in our 2018 Wellness Incentive, an all-inclusive program, focused on helping you achieve the perfect balance of mind, body and spirit through the principles of CREATION Health.  

The Wellness Incentive is broken into two parts:  

Part 1: Completing your biometric screening (15 points) — having four values in range (20 points) — and your health assessment (15 points); total: 50 Points

Part 2: Participating in our CREATION Health programs and activities (50 points)

You earn points that lead to dollars by having your biometric values in range during your screening and completing healthy activities.  Completion of Part 1 and Part 2 will allow you to earn up $250 in addition to a better perspective on your health.

***The rewards deadline for all WebMD and portal activity for 2018 is November 30, 2018. However, all biometric and CREATION Health program data will need to be submitted no later than October 30, 2018 to ensure all metrics are in the system before the deadline. Please keep this in mind as you schedule your appointments for biometric screenings. 

How do I check my points for my Wellness Incentive?

To check your points for your wellness incentive, you must first log into your WebMD account (click on WebMD login on this site). Once you log in, in the top right corner, you'll click on the tab that reads: REWARDS. Here you will find the points you have obtained for each program and activity. Please note, rewards post at different times based on the date of the program/activity.

What is the Health Assessment?

The WebMD Health Assessment combines leading clinical expertise with innovative design to help you understand what's going on with your health and identify areas for improvement. At the end, there are simple personalized steps that you can begin taking right away to live a longer, better life. As with WebMD clinical material, the Health Assessment is clinically validated and follows the guidelines of the U.S. Preventive Services Task Force. Please note, Florida Hospital cannot access or see any of the data you provide. 

How do I complete my Health Assessment?

To complete your Health Assessment, log in to WebMD, click on Healthy Living and select Health Assessment. Please note, the WebMD Health Assessment will become available within 14 days of completing your biometric screening. You must complete your full Health Assessment between January 1, 2018 and October 30, 2018 to receive your incentive points. For completing this activity, you'll earn 15 points. your earned points will appear in your WebMD account by October 30, 2018.

 

 

Biometric Screening

What is a Biometric Screening?

A biometric screening is a quick and easy health exam that provides information about your risk for certain diseases and medical conditions. Knowing your numbers- the results of your biometric screening, is the first step to your health journey because it helps you understand where you can take action to improve your health.

The screening includes body measurements and a simple finger stick to obtain a blood sample. You can choose to fast or not fast before your screening. Fasting is not required, however, a fasted screening will provide more comprehensive results. During your screening, the following measurements are collected:

  • Hemoglobin A1c
  • Cholesterol (total cholesterol, HDL, and LDL)
  • Triglycerides
  • Blood pressure
  • Height and weight to determine Body Mass Index (BMI)

If your numbers are outside of the recommended ranges, you can complete a WebMD telephone coaching call for each missed biometric value. WebMD coaches will provide helpful advice and support as you work toward improving your health.

How do I complete my Biometric Screening?

To help make completing your biometric screening as convenient as possible, there are free screenings that will take place at multiple Florida Hospital locations between January and April 2018. Appointments are required and available now!

You can book your screening in TWO easy steps:

1. Log-in to your WebMD account by visiting CREATIONHealthEmployees.com and click on WebMD Login. Login and select “Know Your Numbers”

2.Find a date and time that is convenient for you at any of our campuses from January through April, 2018. Appointments are in 15-minute increments and must be scheduled at least 48 hours in advance

What is a Primary Care Physician Health Check Form and how do I complete it?

  • The Primary Care Physician Health Check Form allows your doctor to complete your biometric screening if you are unable to attend a biometric screening on your campus (Centra Care locations are unable to complete your screening).
  • After completing your biometric screening with your primary care physician, have your doctor fill out the form in its entirety and submit this form to biometrics@flhosp.org or fax to (407) 200-9231 no later than October 30, 2018. Please note, your consent form must be submitted as well.

How do I earn incentive points for CREATION Health Learn & Earns?

Simple - Attend any of the five CREATION Health Lunch and Learns and have your badge swiped. Points are uploaded on quarterly basis (if not sooner). Check your points by going to your REWARDS tab in your WebMD account and input the date you attended the event. You’ll earn 5pts for each CREATION Health Learn & Earn you attend.

 

How do I earn incentive points for 5K, 10K, ½ Marathon/Marathon, Triathlon, Century Bike Ride

Simple- the endurance races listed above are self-reported. Once you complete your race you’ll be able to go to the REWARDS tab in your WebMD account and input the date you completed your event. You can complete this activity two times for a max reward of 10pts.

 

How do I earn incentive points for MISSION: 5k?

  • To earn incentive points simply register for the event, attend 5 out of the 9 training sessions, complete your race and you’ll earn 15pts.
  • Your earned points will appear in your WebMD account by November 30, 2018.
  • Please note: The cost to register is $31 (registration prices vary)
  • Stay in the know on the go by texting FHMission5K to 412412. Data and message rates may apply.

How do I earn incentive points for The Stress Recovery Effect?

  • To earn incentive points simply register for the event, attend the 1 day course or the 6 week course, and you’ll earn 15pts.
  • Your earned points will appear in your WebMD account by October 30, 2018.
  • Please note: This program is FREE for all Florida Hospital Employees. Employees are invited to bring a friend or family member to attend as well.
    • Family member include a spouse, significant other, adult child and/or friend. 

 

How do I earn incentive points for the Walk the Talk Challenge?

  • To earn incentive points simply register as a participant, enter your tracking (steps, weight, and/or minutes exercised) for 20 days of the challenge and you’ll earn 10pts. You can participate in this activity for a max reward of 20 pts. 
  • Your earned points will appear in your WebMD account by October 30, 2018.
  • Please note: This program is FREE for all Florida Hospital Employees.

 

How do I earn incentive points for a Preventative Care Visit?

Simple - preventative care visits are self-reported. So once you have seen your doctor for a preventive care appointment and/or screening, log into your WebMD account to track this information; only the date of your preventative care visit is required. You can report two visits and you will receive a max of 10 points total for your preventative care visit, regardless of how many preventative care visits you have within the calendar year.

 

What is a WebMD Digital Health Assistant?

The WebMD Digital Health Assistant is an online goal tool that provides participants with a method of setting and tacking personal goals treated to several areas including:

  • Managing Stress
  • Maintaining a Positive Mood
  • Nutrition
  • Exercise
  • Weight Loss
  • Quitting

 

How do I earn incentive points for the WebMD Digital Health Assistant?

  • You will receive 5 points for each goal that is completed for a max of 3 goals/15 points.
  • Your earned points will appear in your WebMD account by November 30, 2018.

 

 

Walk the Talk

What is The Walk the Talk Challenge?

  • The Walk the Talk Challenge is a way for you to get healthy and lead a more active life. Through Walk the Talk core challenges, you can work with a team of your coworkers to see who can walk the farthest, exercise the most, and/or lose the most weight!

 

What is Walk the Talk Challenges and how do I register for a Walk the Talk Core Challenge?

The Walk the Talk challenges are a way for you to get healthy and lead a more active life. While working with a team of your coworkers, you'll see who can walk the farthest, exercise the most, and/or lose the most weight! To register, visit http://www.join.virginpulse.com/walkthetalk and log in. If you haven’t participated in a challenge before, you will need to create an account by providing some basic information. Once you have an account, you’ll see an ad for the Walk the Talk Challenge that is currently open; click on it to join. If there are no system wide core challenges running, you can still log on to participate in bonus challenges (week long) and quick challenges (24 hour) that you and your coworkers can create.

 

How do I join a team for a core challenge?

  • As you register for the Walk the Talk Challenge, you will be prompted to join a team in one of three ways:
    • You can form your own team as a team captain
    • Join an existing team
    • Be assigned to a team randomly
  • If you have received a team code from a team captain, you can enter it in to join a specific team.

 

How do I form my own team?

You must register as a team captain to form your own team. When you are asked how you’d like to join a team, choose “create a new team.” You will then be asked to name your team and begin recruiting teammates.

 

What does a team captain do?

Team captains are an important part of the Walk the Talk challenge. Before the challenge begins, team captains recruit team members, name their team, and help team members who may have questions. During the challenge, team captains help people report their activities, remind team members to log in to the website and track, offer encouragement and motivate, and celebrate success. Team captains may also organize team activities, like lunch time walks.

 

Will anyone be able to see my weight?

No. Your weight will never be shared with anyone. 

 

How do I compete?
You compete by tracking your steps manually or by synching a fitness tracker. You can also convert an activity to steps such as weight lifting, cycling or swimming. 


How do I track my activities?
You may track your activities at any time by visiting the “tracking” tab on the Face Off challenge page. If you are tracking steps, enter in the amount of steps
measured by your pedometer. Otherwise, enter in the amount of time you’ve spent exercising. Make sure you track your activities each day! If you forget
to wear your pedometer, try to estimate using this conversion: 1 mile = 2,000 steps.

How are team standings calculated?
Teams are ranked by the average activity levels of each team member, and the more successful individual teams are, the better your campus will do.

Do I earn incentive points for participating?
Yes! You will be rewarded with 10 points toward your 2018 Wellness Incentive by tracking your steps, for a minimum of 20 days of the 8 week challenge.


Will prizes be awarded?
Yes! Prizes are specific to each challenge. Visit the walk the talk page on this website for details.

CHEE

What is CHEE?

CHEE also known as the CREATION Health Employee Experience is a FUN interactive, paid* 8 hour program that takes a closer look at the eight principles of CREATION Health through a variety of ways! From making healthier lifestyle choices through meal planning or finding fun news ways to be active, CHEE has something for everyone! *All employees will receive paid time for attending.

 

I already know what CREATION stands for, will I benefit from it?

Absolutely! CHEE takes a deeper dive into research based examples and practical applications that you can take back to work and home.

 

Where is CHEE located?

Location for CHEE vary, please check the event calendar for details!

 

Do I get paid to attend?

Yes.  Florida Hospital values your time as an employee and believes you should be compensated for your attendance at CHEE.  For hourly employees, you will be prompted to sign-in to the CREATION Health Employees Cost Center upon arrival.  

 

Do I have to get permission from my supervisor?

We understand that you are a valuable part of your department.  We want to make sure leaders are aware of your attendance for scheduling purposes.

 

What do I wear to CHEE?

Comfortable clothes and sneakers are encouraged for the activity portion and can be worn all day.  For the half day you may want to bring a change of clothes if you are planning on returning to work. 

  

Why is it important for me to attend CHEE if I have already attended EFP in the past?

CHEE takes a deeper dive into the research behind whole body wellness and presents the learner with realistic examples for both work and home. 

 

How can I apply CREATION Health into my role at the hospital?

CREATION Health is a roadmap for a fuller life!  Bettering ourselves personally will not only improve our health, but will also allow us to reach our full potential when we come to work. 

 

Why does Florida Hospital want me to attend CHEE?

Florida Hospital cares about their employees.  CHEE is our hospital’s gift to its employees, and its principles reflect God’s purpose for us to live our lives to the fullest.  

 

Can I bring my spouse or significant other to CHEE?

YES! We want you to spread the CREATION Health message to those around you each and every day. 

 

Is there food provided at CHEE?

Yes!

 

Should I bring anything with me to class?

Just comfortable clothes, sneakers and your badge!

 

Is there parking available?

Yes, there is free parking on each side of the church.

 

Can I attend more than once?

Yes, however we are only able to give you paid time for one session.

 

Can I earn incentive points for participating in CHEE?

Because you are paid* to go through CHEE, it is not eligible for incentive points.  *Hourly employees will be able to clock in/out for the duration of this program.

 

Will I have fun? 

Is that a real question? Of course you’ll have fun- so much you’ll be talking about it for weeks to come!

    • Weight Watchers is a weight management service that surrounds members with support and a variety of innovative tools that enrich the overall experience of losing weight. You can choose to participate in one or both of the following options:
    • Weight Watchers Meetings
      • This package features convenient Weight Watchers meetings in your workplace and unlimited meetings in your local community. It includes access to OnlinePlus. $19.11/month. For those short on time, Weight Watchers now offers Weigh & Go...simply come in and weigh..and GO!
    • OnlinePlus
      • This subscription allows you to easily follow the plan step-by-step entirely online with digital and mobile tools including weight tracker, progress charts, restaurant guides and much more. It is available in two versions with tailored content designed for men and women. $8.48/month

     

    How do I earn incentive points for Weight Watchers?

    • In order to receive points for the wellness incentive, you must register for Weight Watchers using the Florida Hospital benefit. To use the Florida Hospital benefit, you must register by going to www.CREATIONHealthEmployees.com. Locate the Programs Menu and click on Weight Watchers. Once you enter the Weight Watchers portal, use the following credentials to sign up: Employer ID: 10290086, Employer Passcode: ww10290086.
    • To qualify for the wellness incentive, you must be a member of Weight Watchers, either online, Weight Watchers Meetings or Weight Watchers for Diabetes, for a minimum of three consecutive months by October 30, 2018. For completing this program, you’ll earn 15pts.
    • Your earned points will appear in your WebMD account by October 30, 2018.
    • Please note: Employees are invited to bring a friend or family member to attend as well.
      • Family member include a spouse, significant other, adult child and/or friend. 

     

    What is CHIP?

    • CHIP (COMPLETE HEALTH IMPROVEMENT PROGRAM (CHIP) is a scientifically proven lifestyle education program founded on more than 25 years of evidence-based medicine. It is designed to prevent, arrest and even reverse common risk factors for chronic diseases such as obesity, type 2 diabetes, cardiovascular disease and high blood pressure. These improvements are achieved through the adoption of better health habits and lifestyle modifications. CHIP is administered through 18 group lifestyle education sessions over 8 weeks.
    • CHIP can deliver significant health changes in as little as 30 days, including improved blood pressure, cholesterol, triglycerides, BMI, fasting blood sugar, sleep, resilience and depression.

     

    How do I earn incentive points for CHIP?

    • To sign up for CHIP, call (407) 303-4469 or visit the Programs page and select CHIP. You can also find more information at CHIPHealth.com. CHIP costs $250 for Florida Hospital employees and includes tuition, health assessments with lab work, classroom materials, food samples and a fitness center membership. Payment plans are available. Employees are eligible for reimbursement of their $250 after attending 15 of 18 classes. To qualify for wellness incentive points, you must enroll in and complete CHIP before October 30th.
    • To qualify for the wellness incentive, you must enroll in and complete the required number of sessions per CHIP requirements before October 30, 2018.
    • For completing this program, you’ll earn 20pts.
    • Your earned points will appear in your WebMD account by October 30, 2018.
    • Please note: Employees are invited to bring a friend or family member to attend as well.
      • Family member include a spouse, significant other, adult child and/or friend. 

     

    What is the Thrive! (Formerly the Florida Hospital Diabetes Care Program)?

    The Diabetes Care Program is available to help Florida Hospital Employee Health Plan members who have diabetes maintain a standard of care and improve their health. Administered by the Florida Hospital Diabetes Institute (FHDI), this program allows participants to earn enhanced benefits including:

    • Waived copay of one quarterly primary care physician or endocrinologist visit for diabetes management within the Florida Hospital Medical Group (FHMG) network.
    • Waived copay for one dilated eye exam by an ophthalmologist within the Florida Hospital Healthcare System provider network.
    • $10 copay for 90-day supplies of all oral and injectable diabetes medications including insulin syringes, pen needles, brand-specific insulin pump reservoirs and standard infusion sets purchased through the Rx Plus mail order program. Includes brand name if no generic brand exists.
    • Free diabetes and therapeutic lifestyle coaching by an assigned FHDI diabetes educator – registered nurse or registered dietitian.
    • Usual plan benefit for blood glucose monitoring supplies.

     

    How do I earn incentive points for Thrive!?

    • Enrollment is voluntary and free of charge to Florida Hospital, Centra Care, Florida Radiology Imaging, Hospice of the Comforter and FHMG employees and dependents only, who are members of the Florida Hospital Care Advantage health plan. Enrollment is open in July annually. For information on registering and the complete requirements of the program, call the FHDI Diabetes Services Line at (407) 303-2720.
    • To qualify for incentive points, you must be enrolled in Thrive! for a minimum of 3 months through October 30, 2018. Only employees, their spouses and dependents enrolled in the FHCA health plan are eligible to enroll in Thrive!
    • For completing this program, you’ll earn 20pts.
    • Your earned points will appear in your WebMD account by October, 30, 2018.

     

    THE STRESS RECOVERY EFFECT

    How do I register for The Stress Recovery Effect?

    Visit www.CREATIONHealthEmployees.com; click on Events. Once on the Events page, select The Stress Recovery Effect from the Event Type drop down box.

     

    How much does it cost to participate in The Stress Recovery Effect?

    There is no cost to participate in this program.

    • Please note: Employees are invited to bring a friend or family member to attend as well.
      • Family member include a spouse, significant other, adult child and/or friend. 

    Where will the classes be held?

    Classes will be held at various campuses. To find out if your campus is offering The Stress Recovery Effect, visit CREATIONHealthEmployees.com and click on the “Events" tab and select your campus and The Stress Recovery Effect.

     

    How long does the program last?

    The Stress Recovery Effect is 4 hour class.

     

    Will I earn points towards my wellness incentive?

    Yes, participants in The Stress Recovery Effect can earn 15 points for completing the class.
